• To enroll a student at Taylorsville Elementary School, please bring the following items to our office between the hours of 9:00 a.m. and 12:00 p.m. for your child to begin school the next day.
     
    • Birth Certificate
    • Immunization Record
    • Proof of Residency - This must be a gas, water, or electric bill with a parent or guardian's name. If you don't have one of these bills in your new address, we can use a Purchase, Lease, or Rental Agreement as long as the property address is on the document.
     
    During the enrollment process, there are several other forms we will ask you to complete. These include:
     
    • Application for Free or Reduced Meals and Textbooks - Did you know that if you qualify for this assistance, you also help the school?  We encourage you to sign BOTH sections of this form!
    • Criminal Background Check - All Classroom/School Volunteers and Field Trip Chaperones MUST have this form on file in the Office before you can volunteer or go on a field trip. These are good for 1 year.
